The Medical Transport Boat is largest medical boat that was added with the 112 Operator Water Operations DLC. Though slightly more expensive than a Water Ambulance, the Medical Transport Boat provides multiple different advantages that make it a strong contender for forming the backbone of the medical fleet. It has the largest capacity for injured passengers of any vehicle in the game, coupled with a large bonus to medical skills and a reasonably-good speed on the water.

The Medical Transport Boat is the last and largest boat to be unlocked in the Water Operations DLC. This typically happens either 8 days into a career, scenario, or freeplay game, or whenever the first water district is unlocked on the map; whichever comes later.
The Medical Transport Boat has similar properties to a Water Ambulance, but trades a little bit of speed for a lot of passenger capacity and a major boost to medical skill. These features make the Medical Transport Boat a very competitive choice, especially since its price is only about 20% higher than that of a Water Ambulance.
The Medical Transport Boat carries 2 medics and up to 4 injured passengers - the largest capacity of any medical vehicle in the game. It provides a +0.6 Helped Injured Person skill bonus to each of its two medics, speeding up treatment by a significant margin.
The large passenger capacity can be incredibly useful for dealing with mass-casualty incidents on the water, including food poisoning, shootouts with smugglers, and the occasional plane crash. A single Medical Transport Boat can sometimes handle these incidents completely on its own.
The cost of a Medical Transport Boat is only €70,000, which is just €12,000 more than the Water Ambulance - quite worthwhile for all of the advantages listed above. Additionally, the Transport Boat has a top speed of 145 compared to the Water Ambulance's 155 - not a very substantial difference.
If money isn't a serious issue, purchasing Medical Transport Boats to serve as the "mainstay" of the medical fleet is a justifiable decision; but depending on the layout of the map, it could just be overkill.
